ConnectiCare Program Earns Honors

FARMINGTON - HMO ConnectiCare's Better Respiration Equals Asthma Treatment and Health Education (BREATHE) program has won a National Exemplary Practice Award from the American Association of Health Plans (AAHP) and the Washington Business Group on Health (WBGH). The honor recognizes health plans that demonstrates exemplary practices in the management of chronic conditions in its members. The award was presented to ConnectiCare during AAHP's national conference in San Diego.
The BREATHE program demonstrated clinical outcomes that significantly exceeded the annual targets in the categories of hospital admissions and emergency-room events per 1,000 members with asthma.
ConnectiCare members with asthma participating in the program experienced an overall 51-percent decrease in hospital admissions, and a 60-percent decline in ER visits per 1,000 asthmatics since the program's inception in 1996. Controlling asthma through self-management has the potential to save millions of dollars in unnecessary medical expenses in this state each year.
