ConnectiCare Earns Innovation Prizes

FARMINGTON - ConnectiCare will receive six Connecticut Quality Improvement Award (CQIA) silver entry-level CQIA Innovation Prizes for excellence in managing quality improvement. The HMO was selected from a field of 110 CQIA Innovation Prize applications from 60 organizations statewide.
ConnectiCare's award winning programs included: 1) The Birth Expectations Program's two-level early-intervention system that includes a self-administered risk survey to identify pregnant women at high risk for preterm labor; 2) The DiabetiCare Program which provides information, education, and case management to members with diabetes; 3) A program which provides physicians with validated adolescent depression screening tools and a brochure for parents; 4) A depression screening program for members with chronic illnesses; 5) A program to increase the adult influenza immunization rate; and 6) A program to improve overall phone service.
