MIDDLETOWN — Citizens Bank of Connecticut and WTNH-TV in New Haven are seeking applications for their 2004 "Community Champions" program. Nomination categories for this year’s awards are special needs, violence prevention, housing and community health care.

Launched in the fall of 2002, the Community Champions program is designed to recognize and support non-profit organizations for their contributions to Connecticut communities. A new champion is selected each quarter among a range of social service areas.

The successful non-profit applicant receives:
o A $25,000 contribution in unrestricted funds from the Citizens Bank Foundation;

Media coverage from WTNH-TV, including public service announcements and television profiles;

o Volunteer support from both Citizens and WTNH employees;
o Significant public relations support;
o Promotional support highlighting the Community Champion in Citizens Bank branches and on its ATMs; and
o Exposure on both the Citizens and WTNH Web sites.

Special needs is the topic of focus for the first quarter of 2004. A non-profit organization that serves individuals with physical or developmental disabilities will be selected in January. The application deadline for this category is November 7 at 5 p.m.

Applications can be obtained online at www.citizensbank.com, www.wtnh.com or at any Citizens Bank branch. Completed applications should be sent to: Citizens Bank, Public Affairs Department, Attn: Community Champions program, 237 Main Street, Middletown 06457.
